This form is only an inquiry and is subject to review and approval by the Artistic Director of UMW's Theatre Program and the Director of Campus Events & Conference Services. Confirmations will be send to the email address provided on this form. Campus Events & Conference Services staff reserve the right to change the location of a reservation if schedule conditions deem it necessary, at which time the contact person will be notified via phone or email.
I understand that the organization I represent must take full financial responsibility for the loss of or damage to equipment or property related to this event.
Events for orginazations outside of UMW will be required to sign a Facilities Use Agreement, provided by UMW Campus Events & Conference Services Office upon reservation confirmation. Events of 40 or more guests are required to provide proof of insurance per section 173.10 of the Facilities Use Agreement.
