DSS asks students to provide us with documentation about their disability and the likely impact of the disability on their educational experiences.
Examples of documentation include:
- Individualized Education Program (IEP) or 504 Plan
- Medical records such as a patient portal diagnostic summary, office visit summary, audiogram, and vision screening results
- A letter from a healthcare professional
- Neuropsychological assessment report
We recommend that the documentation includes the following information:
- A diagnosis of the disability or health condition
- A description of the current impact and limitations caused by the disability
- The severity of the disability
- Approximate duration of the disability
- A qualified healthcare provider’s identification includes name, title, certification/license, and contact information

All documentation submitted to DSS is kept confidential and separate from academic records under the Family Education Rights and Privacy Act (FERPA). HIPAA privacy rules do not apply to documents submitted to DSS.
