Wednesday, April 14^th, 2021, Safeway Pharmacy will be hosting a clinic for the Johnson and Johnson vaccine.

The clinic is open to everyone 18 years and older. To schedule an appointment please use this link: https://mhealthsystem.com/1581singledose

You will be directed to enter your information and print the form. Please bring this paperwork with you to your appointment. Please also bring your insurance card. There is no charge for the vaccine, but they will bill your insurance for the administration of the vaccine.

Safeway also provides the Moderna vaccine daily.
